  < | A young woman who was connected to the local mental health service for 18 months, usually because of incidents of self harm. There were police convictions for 'against a person', 'against a property', and a misbehaviour in custody. Finally, she took two health workers hostage, was charged and sent to prison for this where one Forensic opinion recorded a possibility of schizophrenia. The interim Order was not extended by the Court, who had received a favourable account from a psychiatrist. Becoming 'informal', she discharged herself from the privately run low secure Unit who were observing her under the interim Order. They had not expected her to leave so soon, and had drawn up no contingency plans for that decision, nor were the police in the picture. They could not hold her. She discharged herself, and later attacked another young woman outside a night club; the woman later dying.
