TW aged 65 could neither read nor write.He could be aggressive and violent to family. He had a disturbed relationship with his aged mother whom he visited regularly. Police and social workers were often involved in that relationship. Statements made indicated TW had strange beliefs pointing to psychotic illness. No Pova. GP recognised mental illness and prescribed medication for schizophrenia. Gp asking for urgent appointment referred to CMHT which was in chaos and had no firm clinical lead to decide intervention. Team maanager had not obtained control. OP appointment were missed . TW stabbed his wife many times. He was found unfit to plead
