Andre da Conceicao is the perpetrator; David Phillips the hostel victim.

 

Andre C. hailed, probably illegally, from Angola. Of his social connections and social support in England and since 1995, little ever became known .

There was therefore no 'inbetween'account of his behaviour or lifestyle that could be called upon. Drugs became accepted as thought to be part of his life style although he never yielded a positive test.

The first community team to formalise his aftercare confirmed a diagnosis of schizophrenia and resettled him in an out of area hostel.

That took him into a different catchment area and into the care of the catchment outreach team which had the connection with the hostel. The lead clinical figure in this adhoc field mental health group, after one initial interview, changed his diagnosis to a diagnosis of drug induced behaviour, then withdrew his maintenance medication, and never personally saw him again.
Andre C. managed to get along for months, superficially alright, and despite aggressive incidents, that allowed the original diagnosis to be forgotten. Misbehaviour, and now not a patient, he lost his place at the hostel although returned again and again to the hostel.
Its policy offered an informal help contact to those who left it.

The final supervising team advised by the clinical lead gradually loosened contact, and finally discharged him without any final review or aftercare.

Some months later, he was admitted to an acute mental illness resource some miles away which proved unable to contain him and he escaped through a narrow window which National Heritage had advised should not be barred.

A muddle then ensued as to who should recapture him - police or staff from the admission ward - who would have to go outside their area to retrieve him from his casual occasional visits to the Hostel.

On a final visit he killed a resident in the hostel.
