Edward Crowley [ FD in the Inquiry report ] stabbed Diego Pineiro, aged 12, 30 times in front of onlookers in Covent Garden, central London, in May 2000.

An unattached, often homeless, middle aged person , FD, has a history of mental disorder, and certain one detaining Order in a mental hospital, moves well away from that family area to London, where for five years he keeps no regular employment nor stays in any particular residence. He is noticed to be harrassing a minor, 'A' , aged twelve, in secondary schooling.
A. is at this time only loosely supervised by a parent. There have been recurrent supervisory problems between 'A' and the mother since he was eight . A request was made to find 'A' a residential school. Fostering is intermittent and social worker decisions kept 'A'at home.

It is difficult to discover what starts, develops into and maintains, an overly friendly contact from FD which raises issues of pursuit, stalking, and paedophilia. Social services protection agencies and the police are involved. FD is seen by a forensic Community nurse, not in any team, who advises - as he finds him - there are no grounds for Mental Health Act hospital detention of FD.

FD puts graffiti about 'A' on the school which 'A'attends. He is charged and given a two year conditional discharge. Charges of harrassment are later laid against FD. He is seen in custody by a diversion forensic psychiatry team. Court is told a full assessment cannot be made, there is no psychiatric requirement, and he is found fit to plead before the Court.

That is the last Psychiatric engagement. Court proceedings drag out for four months and do not come about. FD is never into settled supervision.

FD finds 'A', who is with his elder brother. FD attacks them. 'A' is stabbed and killed.
